Presentation
Two-year history of medial left ankle pain and stiffness, which is exacerbated by physical activity. Although the patient has a prior history of ankle sprains, there has been no recent acute injury. Despite undergoing physical therapy, there has been no significant improvement in his condition.
Patient Data
There is irregularity at the articulation between the middle facet of the subtalar articulation between the calcaneus and talus. There is associated marrow oedema on either side of the synchondrosis compatible with fibrous talocalcaneal coalition.

Case Discussion
In this case, the MRI reveals a fibrous connection between the talus and calcaneus, accompanied by surrounding reactive oedema that is consistent with talocalcaneal coalition. The patient underwent surgical excision of the middle facet coalition without any complications. During the three-week post-operative checkup, the patient exhibited remarkable progress in physical therapy with only occasional use of paracetamol, while radiography revealed appropriate post-surgical changes.
