A history of trauma and ankle pain with intermittent soft tissue swelling.

Osteochondral defect (OCD) grade IV in the upper lateral side of the talus dome with medullary sclerosis and vertical fracture line through the posterior segment of the avulsed bone is seen.
A small bone island in the talus body is also seen
Case Discussion
The case illustrates non-contrast MDCT features of the talus osteochondral defect (OCD) on the lateral side of the talus dome. The usual location of the lesion is on the medial side of the dome. Management and prognosis of the lesion are usually based on Berndt and Harty's classification of osteochondral lesions of the talus.
