Presentation

Low back pain.

Patient Data

Age: 55 years
Gender: Female

Voluminous perineural cyst occupying the spinal canal from L5 to S3-S4 with diameters of 8 x 3 cm, with completely liquid content. Significant bony remodelling of the sacrum secondary to mass effect. The medullary cone and the cauda equina are normal.

Case Discussion

A Tarlov cyst is a dilatation of the subarachnoid space surrounding a spinal nerve root. Most patients are asymptomatic with no demonstrable need for care.

Tarlov cysts are most frequently found at the S2 level, and bone remodelling is a common feature of large cysts.
