Temporal lobe atrophy post herpes simplex encephalitis

Presentation

History of herpes encephalitis, now seizures. Poor short term memory.

Patient Data

Age: 30 years
Gender: Male

FLAIR performed 2 years prior to current scan at the time of acute illness

Gross atrophy of the right temporal lobe (grey and white matter) with ex-vacuo dilatation of the right temporal horn.  Surrounding T2 brightening indicative of gliosis.

Case Discussion

Herpes simplex encephalitis is a devastating infection and if not treated early can cause severe destruction of brain tissue leading to gross atrophy as evidenced by this case.
