A frontal radiograph demonstrates an increase in the scapholunate space on an AP radiograph of the wrist, indicates scapholunate dissociation and often can be associated with rotary subluxation of the scaphoid.

Just to make this sign more clear, for comparison, a normal x-ray of the wrist. Note that there is a much more narrow interval between the scaphoid and the lunate.

Case Discussion

Terry-Thomas sign is an important sign on wrist x-rays that indicate a rupture of the scapholunate ligament leading to the separation of the lunate and scaphoid bones. The injury also causes rotation of the scaphoid bone.
