Presentation

Known congenital adrenal hyperplasia. Lost to follow-up and treatment.

Age: 40 years
Gender: Male

Both testes have heterogeneous echotexture with ill-defined nodular areas of predominantly increased echogenicity, somewhat nodular in appearance and with a rete testis area predominance but involving all the gonads. Trace of hydrocele bilaterally. The epididymis and scrotal wall are unremarkable. 

Case Discussion

In the setting of untreated congenital adrenal hyperplasia, the findings of these mass-like lesions involving both testes are favored to represent testicular adrenal rest tumors (TARTs).
