Testicular atrophy

Diagnosis almost certain

Presentation

Infertility, married for 7 years with no children. No erection, no body hair, bilateral gynecomastia, and penis measuring 3 cm. Testosterone level: 0.89 nmol/L (decreased). The patient had a history of cleft palate repair since childhood

Patient Data

Age: 40 years
Gender: Male
ultrasound

Bilateral testicular atrophy is noted with several macrocalcifications.

The right testis measures 13.8 x 6.8 mm.

The left testis measures 14.8 x 10.4 mm.

No clear internal vascularity is noted.

Case Discussion

The patient had a history of cleft palate repair since childhood, this congenital malformation may be seen in the Kallmann syndrome which may associated with cryptorchidism
