Right testicular painless mass since 15 years ago and recently had increased in size.

Longitudinal and transverse US images of the right testis show a well-circumscribed nonvascular intratesticular mass in the lower aspect of the testis. The surrounding testis is normal.
Case Discussion
The patient's laboratory tests including levels of the tumor marker serum and BHCG were normal.
Physical examination revealed no evidence of lymphadenopathy or gynecomastia.
Regarding the US, clinical, and laboratory findings epidermoid cyst is the main differential diagnosis.
