Testicular epidermoid cyst

Presentation

Palpable lump on right testicle.

Patient Data

Age: 17 years
Gender: Male

The right testicle is normal in size. At the superior aspect of the right testicle, there is a 1.2 x 1.5 x 1.1 cm well-circumscribed mass demonstrating an "onion-skin" (whorled) appearance with alternating layers of hyperechoic and hypoechoic rings. There is no significant color Doppler

Case Discussion

This is a case of pathologically-proven testicular epidermoid cyst. Pathologic examination following a right partial orchiectomy revealed a unilocular cyst containing keratin. Microscopically, the cyst was lined by squamous epithelium without identification of skin appendages or other tissue types. There was background testicular parenchyma demonstrating mature spermatogenesis and no germ cell neoplasia in situ. There was no embryonic type neuroectodermal tumor, germ cell, or non-germ cell malignancies identified. These findings support the diagnosis of an epidermoid cyst, which is a form of prepubertal teratoma.
