Right testicular pain for one day

Right epididymis is enlarged, swollen and twisted with a whirlpool appearance, with decreased vascularity. The right testis shows no internal vascularity and mild irregular outline reflecting subacute ischemia. Minimal turbid septated right hydrocele is noted.
Left testis is normal.
Case Discussion
Features of right subacute testicular ischemia, likely due to testicular torsion. Testicular ischemia can be a sequel to inguinal surgery, testicular torsion, and epididymo-orchitis. If not managed in the first hours of ischemia, in reversible causes like testicular torsion, it will lead to testicular infarction.
