Testicular lipomatosis

Presentation

Left scrotal pain with suspected left-sided varicocele.

Patient Data

Age: 40 years
Gender: Male
Ultrasound

Scattered multiple well-defined non-shadowing, homogeneously hyperechoic (of fat-similar echogenicity) lesions in both testicles, with no detected intra-lesion Doppler color flow.

Otherwise, both testicles show normal size and outline (with no tunica albuginea distortion) and preserved normal Doppler color flow (shown) and pulse wave (not shown).

Case Discussion

The noted above incidental features are likely representing testicular lipomatosis. Left-sided varicoceles were confirmed sonographically (not shown).

Further management may include searching for or correlation with hamartoma tumor syndromes, especially PTEN gene-associated ones.
