Diagnosis almost certain

Presentation

Right testicular pain.

Patient Data

Age: 60 years
Gender: Male
mri

The right testis is seen diffusely enlarged by virtue of a large soft tissue mass lesion measuring about 4.7 x 4.3 x 6.5 cm. It elicits a low signal on T1WI, an intermediate signal on T2WI and restricted diffusion on DWI, and a low signal on ADC showing internal areas of breaking down. Mild right hydrocele.

Enlarged prostate showing heterogeneous central zone (benign prostatic hyperplasia). Clear peripheral zones.

Case Discussion

The patient had a right orchiectomy and histopathology revealed Diffuse large B-cell lymphoma (DLBCL).

Post-operative PET-CT showed bilateral pelvic lymphadenopathy and bilateral adrenal involvement. So this is a secondary involvement of the systemic lymphoma.

Testicular lymphoma maybe also the primary site of extranodal disease (primary testicular lymphoma).
