Presentation

Painless right testicular mass.

Patient Data

Age: 35 years
Gender: Male

Well-circumscribed ovoid right testicular mass isointense to the normal testicular tissue on T1, hypointense on T2 with a punctate calcification of a low signal and restricted diffusion on DWI/ADC. The postcontrast sequences show a heterogeneous enhancement. No extension beyond the tunica vaginalis.

The left testis appears normal.

No significant inguinal lymph nodes.

No distant metastasis or lymphadenopathy was seen on the chest and abdominopelvic CT (not shown).

Case Discussion

The MRI features are most consistent with a malignant testicular tumor.

The patient underwent a right-sided orchiectomy with a histopathological exam that confirmed the diagnosis of testicular seminoma.

Testicular seminomas are a type of germ cell tumor and are considered the most common testicular tumors, accounting for ~45% of all primary testicular tumors.
