Left hemiscrotal pain and swelling for the last 5 days.

The left testis shows an altered shape with ill-defined small hypoechoic foci in the parenchyma along with the absent parenchymal flow. Abnormal contour and echopattern of the left epididymis. Left tunica vaginalis small hydrocele with thin septa. Twisting of the left spermatic cord on grey-scale as well as on colour Doppler.
Normal right testis and epididymis.
Cine-loops align to the left spermatic cord short-axis and show twisting of the spermatic cord. 1st loop runs from the cranial to the caudal direction. The other loops run to and fro direction.
Case Discussion
The patient developed left hemiscrotal pain followed by swelling for the last 5 days. The ultrasound shows the primary finding of testicular torsion which is a twisted spermatic cord. Secondary signs involving the testis favoured infarction which was confirmed with surgical exploration. A left orchidectomy was done.
