Testicular torsion and infarction

Presentation

Right hemiscrotal pain for last 24 hours. Right hemiscrotal swelling confirmed on exam.

Patient Data

Age: 25 years
Gender: Male
ultrasound

Relative enlargement and hypoechogenicity of the right testicle relative to the left. No flow on color Doppler imaging. Findings are compatible with testicular torsion.

Case Discussion

Patient initially presented with right iliac fossa pain for three days.  Ultrasound of abdomen and pelvis done on initial presentation was normal.  Responded to conservative treatment and was symptom free two days before he represented with right hemiscrotal pain. 

He was operated on after a few hours, but unfortunately, the testis was necrosed.
