Ultrasound demonstrates an enlarged, hypoechoic left testis with no internal vascularity. There is a complex left hydrocele. Normal looking testis and a simple hydrocele on the right.
Case Discussion
This is an acute extravaginal testicular torsion (i.e. torsion of the testis before it is fixed down by the tunica vaginalis) which was present at birth. The testis was not viable.
