Testicular trauma - pediatric

Diagnosis almost certain

Presentation

Direct blunt trauma to scrotum.

Patient Data

Age: 10-15 years
Gender: Male
ultrasound

Right testis: Enlarged. Heterogeneous echotexture compatible with hematoma. Medially, there is a hypoechoic cleft compatible with a fracture. No Doppler signal present within testicular parenchyma medial to this cleft. At the superior aspect, the tunica albuginea is not well identified and capsular rupture is suspected.

Left testis: Normal examination. 

Case Discussion

The patient proceeded to urgent scrotal exploration, which confirmed the imaging findings and underwent scrotal repair. A viable testis was present on follow-up ultrasound (not shown) six weeks later.
