Testicular tumour mimic on MRI

Presentation

Large left hydrocele and heterogeneous echo pattern of the left testicle on ultrasound.

Patient Data

Age: 65
Gender: Male
Modality: MRI

MRI showed an apparent T2-hypointense, T1-isointense lesion with irregular borders at the mediastinum testis, that enhanced with gadolinium. The left epididymis was difficult to visualise, probably atrophic. Large left hydrocoele. The right scrotal contents appear normal.

Case Discussion

Open incisional biopsy was performed. Histology showed marked stromal fibrosis and mild chronic non-specific inflammation. No malignancy.
