L5 occult spina bifida (midline bony defect at the posterior arch) is associated with fibrous tissue and a low lying conus.
The conus ends abnormally low at the level of L4.
No evidence of neural foraminal compromise or spinal stenosis.

This case demonstrates radiological features of occulta spina bifida at the L5 level and tethered cord.
