TFCC disc injury and UCCL complete tear (wrist MRI arthrogram)

Presentation
Chronic pain on left wrist for 6 weeks after lifting heavy object at gym. Clinical examination: tenderness on ulnar side of the wrist and painful radial deviation.

Pooling of contrast material at the ulnar side of wrist joint. No leakage into midcarpal apace and DRUJ.

- Early subchondral sclerosis at the distal end of radial bone on T1 and T2.
- Complete UCL tear.
- TFCC complex tear and detachment at the region of the TFCC disc and ulnotriquetral ligament with associated intra-articular contrast pooling at ulnar side of the wrist joint.
- Mild joint effusion in pre-contrast films.
- Intact radioulnar ligament of TFCC without any contrast leakage into DRUJ.
- Intact scapholunate and lunotriquetral ligaments with no contrast leakage into the midcarpal space.
- MRI signal of bone marrow edema at styloid process of the ulna.
