Sail-like opacity within the superior mediastinum and the left upper lung zone.
Oftentimes, the inferior margin of the thymus gland in the pediatric population can be seen as a triangular opacity superimposed on one lung field. This appearance, which is considered normal on a child's chest radiograph, has resemblance to the sail of a boat, thus it is referred to as thymic sail sign.
