The CT demonstrates bilateral and symmetrical enlargement of the extraocular muscles with enhancement, sparing their tendinous insertions. Normal thickness and density of both optic nerves. No intra-or extraconal lesion is seen.
Normal pneumatization of the paranasal sinuses.

The interzygomatic line (a line that connects the anterior edges of the zygomatic bones) is in contact with the posterior margin of both globes, in keeping with bilateral proptosis grade 2.
Case Discussion
Bilateral proptosis with bilateral and symmetrical enlargement with enhanement of the extraocular muscles with bilateral proptosis, indicating probably a thyroid-associated orbitopathy.
