Presentation
Worsening vision. Bilateral (left > right) exophthalmos and eyelid retraction. History of Graves' disease.
Patient Data
There is bilateral exophthalmos, left greater than right.
There is enlargement of the bilateral (left greater than right) inferior rectus muscles and medial rectus muscles. There are streaks of low density within the muscle bellies of the enlarged extraocular muscles, suggestive of fatty infiltration. There is sparing of the muscle tendons.
Case Discussion
This is a case of thyroid-associated orbitopathy. The patient underwent an endoscopic orbital medial and inferior orbital wall decompression, right medial wall decompression, and bilateral frontal sinusotomy with neuronavigation with lateral orbital wall decompression.
