Presentation

Proptosis, hyperthyroidism.

Patient Data

Age: 40
Gender: Female

Extra-ocular muscles, apart from lateral recti, show bilateral symmetrical diffuse bulkiness of their bellies with increased fatty content and relative preservation of their tendons.

Bilateral exophthalmos; both globes seen anterior to the level of the anterior tip of zygomatic bones.

Prominent retro-orbital fat bilaterally.

Case Discussion

CT through the orbits of a patient with known thyroid-associated orbitopathy, demonstrating enlargement of the superior, inferior and medial rectus muscles. The muscle bellies are enlarged with sparing of the tendinous insertions in the appearance of a traditional coca-cola bottle. The order of involvement can be remembered using the mnemonic I'M SLOW
