Presentation
Right shoulder swelling. Known metastatic thyroid carcinoma to lungs and bones.
Patient Data
There is a large expansile destructive osteolytic lesion seen in the distal right clavicle with a soft tissue mass measuring about 8 x 6 cm.
Right thyroid lobe calcified mass is heterogeneously enhancing with mild displacement, and slight compression, of the trachea.
Multiple variably-sized suspicious pulmonary nodules are seen scattered within both lungs, more seen basally, the largest in the medial basal segment of the right lower lobe.
A destructive osseous lesion is seen implicating the distal portion of the right clavicle with associated heterogeneously enhancing soft tissue component.
A sclerotic lesion within the left humeral head is indeterminate.
Case Discussion
Presented with right shoulder swelling and imaging showed a destructive lytic lesion of the clavicle and multiple lung nodules. Radiological work-up, including thyroid FNAC, confirmed the diagnosis of metastatic thyroid cancer.
