Patient Data
There is severe radiological proptosis bilaterally. There is diffuse, fusiform thickening of the inferior rectus, medial rectus and superior rectus muscles bilaterally, The inferior and medial recti most effected, with the superior recti less so. There is very minimal fat stranding surrounding the aforementioned affected extra-ocular muscles. There is mild increase in the volume of intraorbital fat also, which is likely a consequence of thyroid eye disease. No focal intra or extraconal mass or collection identified.
No bony abnormality identified. Imaged paranasal sinuses are clear other than for mild left anterior ethmoid air cells disease.
Imaged brain and intracranial contents are unremarkable.
Conclusion
Findings in keeping with severe thyroid orbitopathy.
Case Discussion
This patient had an established diagnosis of Graves disease, and has typical appearances of thyroid orbitopathy.
