Fall 4 days ago. Unable to weight bear. Normal x-ray.
Patient Data
Knee joint effusion. No haemarthrosis.
Osteochondral injury through the lateral tibial plateau with an undisplaced fracture line extending deep to the tibial intercondylar eminence to the central portion of the medial tibial plateau and also extending the entire anteroposterior depth.
Inner margin radial tear of the posterior medial meniscus. Lateral meniscus is intact. Anterior cruciate ligament and posterior cruciate ligament are intact. Medial collateral ligament is intact. High signal of the distal semimembranosus and posteromedial meniscocapsular fibres. High signal of the proximal fibular collateral ligament. Remainder of the lateral collateral ligament complex appears intact.
Case Discussion
Tibial intercondylar eminence fractures are not so uncommon, more commonly involving bony avulsion of the anterior cruciate ligament insertion. In this case, the fracture pattern is less common in that it involves both the anterior and posterior parts of the intercondylar eminence extending deep to both the anterior and posterior cruciate ligament insertions.
