The radiographs in the emergency setting show a slight irregularity of the tibial spines and an increase in soft tissue in the suprapatellar topography.

In control radiographs, the irregularity and loss of the usual morphology of the tibial spines become more evident, findings compatible with fracture.

Case Discussion

The patient suffered a motorcycle injury with his left lower limb hitting the ground. The pathology could already be suspected from the initial x-rays taken in the emergency department and follow-up x-rays due to joint instability eventually confirmed the diagnosis. Conservative treatment was chosen.

Fractures of the tibial spine are rare in the general population, affecting children and adolescents more due to skeletal immaturity. This type of fracture may have associated injuries such as meniscal lesions, concomitant ACL lesions, chondral lesions, bone bruises and tibial plateau fractures.
