Two medial malleolar screws, endobuttons and two transverse syndesmotic bone tunnels are in keeping with ORIF and syndesmotic stabilization. The medial malleolar fracture is united. The ankle is normally aligned.
Case Discussion
The TightRope® device is a contemporary orthopedic internal fixation system for distal tibiofibular syndesmotic injuries, used instead of syndesmotic screws. Unlike syndesmotic screw, it does not need to be removed thereby preventing a second operation. Also earlier weight-bearing can be achieved. A medial ankle incision doesn't need to be made which reduces morbidity.
The device is a braided polyethylene cord (radiolucent) that is anchored medially and laterally with metallic endobuttons (radiopaque).
