Total anomalous pulmonary venous return

There is cardiomegaly with an abnormal ovoid density at the superior mediastinum resulting to a "snowman" or "figure of 8" appearance of the cardiomediastinal shadow on frontal projection.
Increased pulmonary vascularity (pulmonary plethora) is also demonstrated.
