Green arrow points to the tracheal diverticulum on scanogram.
Purple arrows point to a tiny point of communication of this cyst to the trachea lumen at the most proximal part of the cyst, more visible with windowing.
Case Discussion
A tracheal diverticulum is usually an incidental finding and mostly lies to the right where there is no esophagus supporting the paratracheal tissue. A direct connection with the trachea may not always be shown on CT.
