Transient hip osteoporosis

Case contributed by Dr Bahman Rasuli

Presentation

Recurrent right hip pain.

Patient Data

Age: 35 years
Gender: Female
MRI

Abnormal signal intensity involving the right femoral head and neck displays low T1 and bright STIR signal with no structural collapse or subchondral fracture.

A mild joint effusion also is noted.

Case Discussion

Differential diagnosis includes avascular necrosis of femoral head which includes also subchondral abnormal signal or structural deformity. The hypothesis that transient hip osteoporosis represents an early reversible stage of AVN is controversial.
