Presentation
Twisting injury of knee.
Patient Data
Osteochondral injury involving medial articular facet of patella with a near full thickening cartilage defect.
Osteochondral injury of lateral femoral condyle.
Loose body at the anteromedial aspect of medial femoral condyle.
Small bone contusions with marrow edema also seen in medial femoral and tibial condyle.
Intermediate to high grade tear of mid-segment and femoral attachment of medial patellofemoral ligament.
Low grade sprain of tibial attachment and mid-segment of anterior cruciate ligament.
Low grade sprain of mid segment and femoral attachment of medial collateral ligament.
Low grade sprain of mid segment of lateral patellofemoral ligament.
Linear hyperintensity seen in posterior horns of medial menisci not intersecting with articular margins (grade 2 signal) – likely intrasubstance tear not reaching up to surface.
Mild joint effusion with MCL bursitis.
Case Discussion
Osteochondral injury involving medial articular facet of the patella and lateral femoral condyle. Together with an intermediate to high-grade tear of mid-segment and femoral attachment of medial patellofemoral ligament suggest transient patellar dislocation.
