Presentation

Headache.

Patient Data

Age: 45 years
Gender: Female

Right posterior parietal-occipital large extra-axial space-occupying lesion. It measures 10 x 10 cm in max axial dimensions. It elicits a low signal on T1 WI and a heterogeneous signal on T2 WI, being predominantly of high signal with internal foci of low signal. It shows non-homogenous post-contrast enhancement. The mass is surrounded by peritumoral cysts, prominent vessels, and vasogenic edema signal. The mass exerts a positive mass effect in the form of lateral displacement of the parietal lobe and compression of the right lateral ventricle with contralateral midline shift.

Pathology after excision: Transitional meningioma, WHO grade I.

Case Discussion

Transitional meningiomas are also known as mixed meningiomas because they have components of meningothelial and fibrous subtypes of meningiomas, hence it shows has a heterogeneous appearance on T2WI and a heterogeneous post-contrast enhancement.
