Transurethral needle insertionn - self harm

Presentation

Past history of self harm. Has recently inserted 4 large needles into the urethra. Now has gross hematuria.

Patient Data

Age: 50 years
Gender: Female

4 large metallic needles traversing the urethra to enter the bladder.  Penetration of the bladder into the peritoneal cavity impinging on small bowel although there is no free gas to indicate bowel perforation.  Minimal hematoma.  Note a 5th needle in the left adnexa (arrow).  This is the remnant of a previous self-harm episode.
