Transurethral needle insertion: self harm

Presentation

PH of self harm. Has recently inserted 4 large needles into the urethra. Now has gross haematuria

Patient Data

Age: 50
Gender: Female

4 large metallic needles traversing the urethra to enter the bladder.  Penetration of the bladder into the peritoneal cavity impinging on small bowel although there is no free gas to indicate bowel perforation.  Minimal hematoma.  Note a 5th needle in the left adnexum (arrow).  This is the remnant of a previous self harm episode.

It is surprising that one could self perform such a procedure, let alone the pain that must have been intense.
