Transverse sinus thrombosis

Presentation

Frontal headache, right eye lateral gaze.

Patient Data

Age: 20 years
Gender: Female
mri

There is asymmetry of both transverse sinuses, right larger than the left.

The right transverse sinus appears hyperintense on T1 and FLAIR sequences with absent flow void on T2 sequence. It demonstrates blooming artefact on Gradient echo sequence. Contrast studies show filling defects almost completely filling the right transverse sinus.

No evidence of intraparenchymal hemorrhage or infarct is seen.

Case Discussion

Dural venous sinus thrombosis is commoner in females especially those on oral contraceptive pills. The clinical presentation is non specific including headaches, cranial nerve deficits, seizures or even coma. Venous infarction and or hemorrhage may also occur.

This case illustrates right transverse sinus thrombosis in a young female, though history of oral contraceptive use could not be ascertained.
