Trapezium exostosis

Case contributed by Dr Roberto Schubert

Incidental finding during MRI to rule out an occult wrist fracture.

Patient Data

Age: 40
Gender: Male

There is an unusual distal bony process of the os trapezium in continuation with the volar trapezial ridge. Don't think it's an osteophyte, for there is no osteoarthritis of the trapezio-metacarpal joint. Differential: developmental variant, enthesiophyte at the flexor pollicis brevis origin, osteochondroma (only 1 literature report to date). Let's call it trapezium cornutum for the horn-like appearance on the coronal sections.

Comments welcome.
