Traumatic liver injury with ruptured intra-hepatic IVC

Presentation

The patient presented to the emergency room as a case of blunt abdominal trauma, At presentation the BP was 93/65 with heart rate of about 105 and tachypnea (25/min), On examination his abdomen was rigid and tender.

Gender: Male

Contrast-enhanced axial, coronal and sagittal CT images of the abdomen and pelvis show large hepatic laceration with disruption of more than 50% of the hepatic parenchyma with active extravasation of the contrast material from the intra-hepatic IVC into the peritoneum.

Case Discussion

Blunt abdominal trauma with significant liver laceration and major vascular injury where the contained or active nature of the bleeding into the peritoneum has a significant role in the patient's survival.

This classifies as an AAST grade V liver laceration.
