Presentation

Sustained a fall with head injury, soft tissue damage around the right orbit. ?IC bleeding

Patient Data

Age: 70 years
Gender: Male

Thin, linear sulcal hyperdensities noted in the left parietal region affecting the cingulate sulcus too. 

Soft tissue stranding around the right orbit confirms the site of the primary impact. 

Marked supratentorial hydrocephalus, shunt in situ - unchanged compared to available prior exam (not shown).

Case Discussion

Typical traumatic subarachnoid hemorrhage, distribution in line with contrecoup mechanism. As it is far from any potential large vascular/aneurysmal source this option is not a potential differential diagnosis, thus no IV contrast was given. Conservative management was uneventful, and the hemorrhage was stable on follow-up.
