Traumatic urethral injury

Presentation

Initially voiding difficulty followed by complete urinary obstruction after road traffic accident.

Patient Data

Age: 30 years
Gender: Male
x-ray

Minimally displaced fractures of superior and inferior rami of right pubic bone and vertical fracture of the left sacral ala.

Fluoroscopy

Retrograde UG: The whole length of the penile and bulbar urethra is well-outlined but an abrupt cut-off effect and complete obstruction are noted at the level of membranous urethra.

Antegrade UG: Urinary bladder is well-outlined via a supra-pubic catheter. On stress for micturition, a prostatic urethra is partially outlined with complete obstruction at the junction of the prostatic urethra with the membranous urethra. 

N.B. no local contrast extravasation is seen.

Case Discussion

This case demonstrates features of traumatic urethral injury of the membranous part of the urethra due to multiple fractures of pelvic bone. This was confirmed on cystoscopy.
