Seizures, ethanol dependence, and mild hyponatremia.

MRI Brain

Central pontine T2 hyperintensity and diffusion restriction with "trident" configuration, with peripheral sparing, consistent with osmotic demyelination.

Severe supratentorial chronic ischemic change.
