Right and left vertebral arteries course leftward at the left pontomedullary junction into the left cerebellopontine angle.

Tortuous vertebro-basilar artery coming into contact with and deforming the left trigeminal nerve root entry zone.

Multiple T2/FLAIR high-signal intensity foci without diffusion restriction in bilateral cerebral periventricular white matter suggest leukoaraiosis.

Case Discussion

The features suggest left-side trigeminal neuralgia caused by vertebrobasilar artery compression.
