A well-circumscribed extra-axial dumble-shaped mass, extending along the course of the right trigeminal nerve from its origin from the pons to the Meckel's cave. It appears hypointense on T1WI, hyperintense on FLAIR and T2WI with mild to moderate heterogeneous enhancement following IV contrast. The mass compresses on the pons, midbrain, right middle cerebellar peduncle, right temporal lobe with cerebellar tonsillar herniation. Dilatation of the supratentorial ventricular system with periventricular interstitial edema due to transependymal resorption of the CSF, indicating an obstructive hydrocephalus.

Case Discussion

MRI features highly suggestive of a trigeminal schwannoma
