Trimalleolar equivalent fracture

Presentation

Rolled ankle ?fracture.

Patient Data

Age: 25 years
Gender: Male
X-ray

Mildly displaced oblique distal fibular fracture (Weber B). Ankle joint malalignment with lateral talar shift and medial clear space widening from presumed deltoid ligament disruption. Posterior malleolus fracture.

Case Discussion

This is an unstable ankle fracture with the widened medial clear space and lateral talar shift indicative of deltoid ligament rupture, essentially making this injury the equivalent of a trimalleolar fracture.
