Triplane fracture (Salter Harris IV) of the distal tibia with intra-articular involvement and extension through epiphyses, physis and into the tibial metaphysis. There is mild displacement of the fracture fragments.

There is a triplane (Salter-Harris IV) fracture in the distal left tibia with the fracture line extending from the posterior aspect of the distal tibial metaphysis, intersecting the mid-point of the physis and traversing horizontally laterally through the physis, and vertically through the epiphysis. The fractured epiphysis is displaced posterolaterally.
