Triplex kidney with ectopic ureterocele

Triplex collecting system in the left kidney. The upper moiety is markedly distended and has a very tortuous and dilated ureter terminating in a ureterocoele, appearing ectopic in the region of the prostate/bladder neck. No contrast excretion into this moiety. Two smaller normal caliber moieties are present, one in the mid kidney and the other in the lower pole and these opacify normally. Simple cysts are present in this kidney and there are 2 calculi measuring 4 and 5 mm in the lower pole.
Case Discussion
Triplex kidneys are very rare with only ~100 reported in the literature 1. This patient's left upper quadrant mass is the very distended upper moiety.
