Presentation
Football trauma, pain and tenderness on the dorsal wrist.
Patient Data

There is a small bone fragment at the dorsum of the triquetrum seen only on the lateral view.
No abnormality is seen on the AP view. The pisiform bone overlies the triquetrum in the AP view and obscure the fracture.

CT shows triquetral fracture with avulsion from the dorsal aspect of triquetrum.
Case Discussion
Fractures of the triquetrum usually occur from forced hyperflexion.
Triquetral fractures are the second most common carpal fractures next to scaphoid fractures.
On xray dorsal triquetral fractures seen only on the lateral projection since the pisiform usually overlies and obscures the triquetrum on the AP projection of the wrist.
