Presentation
Severe headache and blurred vision. There is a history of treated bacterial meningitis a few months ago.
Patient Data
MR reveals severe dilatation of lateral and third ventricles with periventricular transependymal edema.
On midsagittal T2 DRIVE images, there is a horizontal web at the upper third of the aqueducts.
There is also effacement of cortical sulci in bilateral cerebral hemispheres due to diffuse brain edema.
The fourth ventricle is normal in size. The tectal plate is unremarkable.
Case Discussion
Hydrocephalus due to aqueductal web stenosis may occur as a result of bacterial or viral meningoencehalitis.
Midsagittal thin slice T2 weighted images very helpful in such cases.
