Presentation

Cough and shortness of breath

Patient Data

Age: 20 years
Gender: Male

Extensive tiny nodules predominantly in centrilobular distribution are seen in the bilateral lung with no lobar preference.

Follow up CT after 3 months

ct

After treatment, extensive tiny nodules have a complete resolution in both lungs.

Case Discussion

The patient had eosinophilic leukocytosis with absolute eosinophil count (AEC) was 32,000 cells/mm3. The serum IgE level was high (6766 KIU/L). The eosinophilia FISH panel report was negative to rule out neoplastic etiology.

After the treatment with steroid and DEC (diethylcarbamazine), his symptom resolved and follow up CT after 3 months shows complete resolution of extensive tiny nodules.

The close differential diagnosis in indian subcontinent includes military tuberculosis.
