Tubo-ovarian abscess

Presentation

55 year-old with abdominal pain and clinically palpable pelvic mass

Age: 55 years old
Gender: Female
MRI

Ultrasound performed by gynaecologist prior to MRI request - query degenerative fibroid

Multi-septated thick walled mass in the left side of the pelvis, displacing the uterus medially.  This contains focal cystic filled spaces, which exhibit diffusion restriction.  The wall and septae of the mass avidly enhance. Perilesional free fluid.  Ovary not separately identified.

Case Discussion

The appearances of the left sided pelvic mass are consistent with a tubo-ovarian abscess.  The diffusion restriction of the internal contents are similar to those observed in other body systems, such as the brain, in the context of an abscess.
